CEP Transcripts

To request an official copy of your CEP transcript, please download and complete the CEP Transcript Request Form. You may submit this form by mail or fax, along with a payment of $5.00 per transcript requested.

To verify your CEP service-learning hours with your school, providing them with a copy of your Student Evaluation & Service-Learning Credit form should be sufficient.
