Book Order

INSTRUCTIONS:

• Complete the form once for each book you need to order.
  

• You must provide accurate ISBN numbers. Without this specific identification we cannot guarantee receipt of the edition you request.

• Please be certain that the books you are ordering are both in print and available. We cannot be responsible for books that are out of print. If you want a rare or out of print book, you must locate the materials, place the order yourself, and request reimbursement.

• CTD book orders are placed through Lake Cook Distributors, Barnes & Noble, and Amazon. Please check with these vendors for your book information.

• Some math, science, and technical books may not be listed on the Lake Cook website. However, as long as the correct ISBN number is provided and the book is still in print, your order will be placed.

• Be sure to adhere to the following spending limits:

• Apogee, Spectrum, Equinox 3-week sessions: up to $110 total per student including supplies, books, course pack, copies, and field trips ($140 for courses with lab fees)

• Leapfrog and Apogee One-Week: up to $25 total per student including supplies, books, course pack, and copies.
