Specific Supply Request

Classes are allotted a maximum dollar amount per student (Leapfrog/Apogee One-Week – up to $25; Apogee, Spectrum, Equinox – up to $110; courses with a lab fee* - up to $140) to spend on:

• supplies
• books
• course pack copies
• field trips

Please keep the budget in mind when choosing your supplies and only request what you need and will use. We keep track of expenditures for each course and will contact you if your supply, book, course pack and field trip requests exceed the funds allotted to you.

This form is only for requesting class specific, specialty items. Please complete the general supply form to request office supplies such as paper, pens, pencils, etc.

Please include detailed information about the following (when applicable) for each item:

• vendor information (i.e. company name and web site or catalog)
• item number
• store/supplier (preferably in the Evanston area)

 

Providing us with accurate and thorough information will make it easier to obtain your supplies in a timely manner.

NOTES RE: Unusual/Specialty Supplies (i.e. difficult to find) –

• If any of the supplies you require for your course are unavailable through a catalog and cannot be found/purchased easily at a store in the Evanston area, the best option is to purchase those supplies on your own and submit your receipts along with the required reimbursement forms.
• Please be advised:
  • The reimbursement process takes up to 8 weeks once forms are submitted.
  • Northwestern University will NOT reimburse for tax. Make sure to bring the CTD tax exempt letter when making purchases (see reimbursement forms packet).
  • A CTD program coordinator must approve all purchases before you shop.
  • All original receipts must be submitted with the required reimbursement forms (see reimbursement forms packet).
