In addition to the completed application, please prepare the following materials:
- Proof of prerequisite course completion, if applicable (transcript or letter from the teacher). See course descriptions for prerequisites.
- Two teacher recommendations from teachers familiar with your work in the relevant academic area.
- Most recent school report card or transcript indicating exceptional performance in the subject area of the Summer Program course for which you are applying.
- Most recent test scores, if available, on a nationally normed, grade level achievement test for the academic area for which you are applying.
- Graded writing assignment for the following courses: all English and writing courses, history courses and social science courses.
- A 250-word essay completely conceived of and written by the applicant on the following topic: Bill Cosby said, “Anyone can dabble, but once you've made that commitment, your blood has that particular thing in it, and it's very hard for people to stop you." What are you passionate about as a citizen of the world? How will that passion help you get the most out of your Equinox experience and achieve your larger goals?

Please include tuition deposit of $200 per session (plus $50 late fee if applying after May 14, 2012) and non-refundable application fee of $60. Only one application fee is required per student, regardless of the number of courses for which a student applies. - Financial Aid Application, if applicable, and all supplementary documentation. - TOEFL or SLEP scores if you are a non-U.S. citizen whose first language is not English.
